When someone uses SMTH in texting, you can answer that if they have written "something", you can answer. For example, if a friend says, "I need to tell you SMTH," You can answer, "Sure, what's this?" Understanding that SMTH simply stands for "something" helps keep the conversation even. If you are unsure, you can politely ask for clarification. Since it is a normal slang, most people who recite regularly, naturally, identify Smith and react easily.
